脾切除对肝豆状核变性合并脾亢病人肝肾功能的影响

目的:观察脾切除对肝豆状核变性(肝豆)合并脾功能亢进(脾亢)病人肝肾功能的影响.方法:回顾分析31例肝豆病人术前1 d与术后2 w肝肾功能的变化.结果:肝豆病人病理性脾切除术后2 w,受损的肝功能均有不同程度的改善,而肾功能并无损害.结论:肝豆合并脾亢行脾切除手术是安全有效的,术后肝功能改善(P<0.05~0.01),肾功能不受损(P>0.05).持续驱铜和加强围手术期保肝治疗是手术成功的关键.

Effects of pathologic splenectomy for Wilson's disease complicated with hypersplenism on liver and renal functions

Objective:To observe the effects of splenectomy for Wilson's disease(WD) on liver and renal functions complicated with hypersplenism.Methods:From January 1996 to October 2002,31 patients with WD complicated by hypersplenism who had undergone splenectomy were analyzed retrospectively,comparing the changes of preoperative liver and renal functions with postoperative ones.Results:After resection of the pathologic spleen with WD,the functions of the injured liver were partially improved and no impairments of renal fundtions occurred.Conclusion:Splenectomy is a secure and reasonable therapy for the patients with WD complicated by hypersplenism. Hypersplenism was eliminated and liver function was improved as well as no impairment of renal function was occurred postoperatively. Continuing decopperization and enhancing support of liver function were the key of successful operation during perioperation.
