摘    要:目的:探讨细胞间粘附分子-1 (ICAM-1 ) 与胎儿生长受限(FGR) 发病的关系。方法: 对2001年1月~2003年12月间38例胎儿生长受限孕妇(FGR组) 和40例正常妊娠孕妇(对照组) 分娩后的胎盘和脐带组织进行分析, 采用免疫组织化学法(免疫组化) 测定两组孕妇分娩后的胎盘和脐带组织细胞间粘附分子-1 (ICAM-1 ) 的表达强度。结果: ①FGR组胎盘绒毛血管内皮细胞中ICAM-1的表达强度为73 68% (28例), 与正常组30% (12例) 比较显著增高, 差异有显著性意义(χ2 =14 .88, P<0 .01); FGR组脐带的脐血管内皮细胞中ICAM-1阳性表达为68. 42% (26例) 与正常组27 .5% (11例) 比较差异有显著性意义(χ2 =13 09, P<0 .01)。②FGR组胎盘重量(464g±57g) 较正常对照组(570g±72g)显著下降(P<0 .01)。③FGR组新生儿体重(2 212 .50g±188 .37g) 较对照组(3 309. 38g±400 .69g) 显著下降(P<0 .01)。结论: 细胞间粘附分子-1 (ICAM-1) 表达的增强在FGR发病中可能具有重要意义。ICAM-1与胎盘的免疫病理损伤有关, 可能是FGR发病机制中的一个重要环节。

Abstract:Objective:To explore the relationship between intercellular adhesion molecule-1 (ICAM-1) expression and incidence of fetal growth restriction (FGR). Methods:Placenta and umbilicus of 38 pregnant women with FGR and 40 normal controls from Jan 2001 to Dec 2003 were analyzed. The ICAM-1 expressions in both groups were determined by immunohistochemistry assay. Results:①ICAM-1 expressions in placenta (73.68%) and umbilicus tissues (68.24%) in FGR group were significantly higher than that in placenta (30%) and umbilicus tissues (27.5%) in controls ( χ 2=14.88, P<0.01;χ 2=13.09, P <0.01). ②The placenta weight in FGR group 〔(464±57)g〕 were significantly lower than that in normal controls 〔(570±72)g〕 ( P <0.01). ③The neonates' weight in FGR group 〔(2 212.50±188.37)g〕 were significantly lower than that in normal controls 〔(3 309.38±400.69)g〕 ( P <0.01).Conclusion:ICAM-1 expression enhancement should play an important role in incidence of FGR, which could be related to immune pathological changes of placenta, might be a key of pathogenesis of FGR.
