各型麻风病人皮肤单核细胞HLA－DR抗原表达的研究

应用免疫细胞化学方法观察多菌型瘤型麻风（ＬＬ）１３例，界线类偏瘤型麻风（ＢＬ）３例，界线类麻风（ＢＢ）２例，界线类偏结核样型麻风〔ＢＴ）４例，结核样型麻风（ＴＴ）２例以及５例正常人皮肤水泡液内单核细胞ＨＬＡ－ＤＲ抗原表达。结果发现，正常对照组单核细胞ＨＬＡ－ＤＲ表达率明显高于病人组（Ｐ＜０．０１）；各病人组间ＨＬＡ－ＤＲ表达率亦有显著性差别（Ｐ＜０．０１和Ｐ＜０．０５），其中以ＬＬ／ＢＬ组最低，依次为ＢＢ组、ＴＴ／ＢＴ组。提示麻风，特别是ＬＬ病人对麻风杆菌缺乏特异的细胞免疫可能与病人单核细胞ＨＬＡ－ＤＲ抗原表达减少，影响其向Ｔ细胞传递抗原关。

STUDY OF THE HLA-DR EXPRESSION BY MONOCYTES IN THE SKIN BLISTERSOF DIFFERENT TYPES OF LEPROSY

he expression of HLA-DR antigen by the monocytes within the skin blisters of 13 pa- tients with LL, 3 BL, 4 BB, 2 BT , 2 TT and 5 normal controls was studied by immunocy- tochemical methods. The positive rate of HLA-DR positive reaction of monocytes in the nor- mal group was significantly higher than in every other group of patients (P<0.01 ). On the other hand ,the expression of if HLA-DR antigen by monocytes among the different groups of patients was also signicantly different (P<0.01 and P<0.05). From TT/BT to LL/BL ,the expression of HLA-DR antigen decreased gradually which was consistent with the phe- nomenon of the immune spectrum in leprosy. Because HLA-DR antigen on the monocytes plays an important role in delivering processed antigen from monocytes to T cells , we sug- gested that its decrease might be one of the important reasons for cell mediated immunity de- pression in leprosy patients,especially LL patients.
