大肠癌致肠梗阻的外科处理

目的：探讨大肠癌致肠梗阻的外科处理方法。方法：分析2005年至2010年间62例大肠癌致肠梗阻的外科治疗资料。结果：62例病人,Ⅰ期行右半结肠切除9例;Ⅰ期行横结肠癌切除4例;Ⅰ期行左半结肠或直肠上段癌切除,近端结肠造口,关闭远端结肠或直肠备Ⅱ期吻合47例。肿瘤无法切除行乙状结肠造口2例（直肠癌）。术后并发症发生率12.9%（8/6 2）,围手术期病死率3.2%（2/6 2）。结论：重视对结肠癌致肠梗阻的认识。合理选择手术方式,做好围手术期处理是减少并发症,提高疗效的重要措施。

The Surgical Treatment of Large Bowel Obstruction Caused by Colorectal Carcinoma

Objective： To study the surgical treatment of large bowel obstruction caused by colorectal carcinoma.Method： Retrospective analysis of the experience of surgical treatment for the large bowel obstruction caused by colorectal carcinoma in 62 patients from 2005 to 2010 was performed.Result： In 62 patients,the right hemicolectomy was performed in 9 patients;left hemicolectomy with proximal colon fistulization was performed in 47 patients;transvercolectomy was performed in 4 patients;sigmoid fistulization was performed in 2 patients for their rectum carcinoma couldn’t be resected.The postoperative complication rate was 12.9%（8/62）,and perioperative mortality rate was 3.2%（2/62）.Conclusion：More attention should be paid to large bowel obstruction caused by colorectal carcinoma.It’s the key to select rational colectomy and appropriate perioperative management for reducing both complication and mortality rate.
