| 摘 要： | 目的观察3种社区康复服务模式对恢复期脑卒中偏瘫患者的影响。方法将87例符合入选标准的脑卒中患者随机分成康复训练组29例、训练强化组30例、随访指导组28例,采用简化Fugl-Meyer运动功能量表评定肢体运动功能,改良Barthel指数评定日常生活活动能力,分别在入组时、康复服务3个月末、6个月末时进行评价。结果各组患者入选时简化Fugl-Meyer运动功能量表、改良Barthel指数评分无显著性差异;在康复服务3个月末时,康复训练组和训练强化组简化Fugl-Meyer运动功能量表、改良Barthel指数评分均有明显改善(P<0.01),随访指导组简化Fugl-Meyer运动功能量表亦有改善,但无显著性差异(P>0.05),改良Barthel指数评分有所改善,程度不及前两组。康复服务6个月末时,训练强化组2个量表评分进一步改善,康复训练组改善幅度减缓,随访指导组量表评分无改善(P>0.05)。结论恢复期脑卒中偏瘫患者在社区接受系统、规范、持续的康复服务,能有效改善肢体运动功能,提高日常生活活动能力。
