早期乳腺癌60例保乳治疗效果的临床分析

目的总结早期乳腺癌施行保乳治疗的效果.方法回顾分析60例早期乳腺癌患者,在接受保留乳房手术、术后辅助放化疗及内分泌治疗后的生存率及乳房的外形.结果全组病人5a总生存率91.7%,5a无病生存率86.7%.局部复发4例.远处转移6例,转移部位：肺3例,骨2例,肝1例,其中5例死于肿瘤转移.对生存的55例患者进行治疗后乳房外形的评价,运用放射治疗联合中心（JCRT）标准,优、良、中、差分别为22.9%、45.1%、26.9%、5.1%.化、放疗序贯进行乳房外形优良者占69.5%,化、放疗同时进行则为68.4%,差异无统计学意义（P〉0.05）.结论保乳治疗对于早期乳腺癌患者而言安全可行,治疗后有良好的乳房外形.放化疗顺序对乳房美容效果无明显影响.

Clinical Effects of Breast Conserving Treatment in 60 Early Breast Cancer Patients

Objective To summarize the effect of the breast conserving treatment for early breast cancer.Methods The survival rate and cosmetic results were retrospectively analyzed for 60 patients with early stage breast cancer who received breast conserving surgery,post-operative chemotherapy,radiotherapy and hor-monotherapy.Results Among the patients,5-year survival rate was 91.7% and disease free survival rate was 86.7%.4 cases experienced local relapse.Distant metastases were found in 6 cases with 3 cases in lung,2 cases in bone,1 case in liver.The cosmetic results were evaluated according to the JCRT（Joint Center for Radiation Therapy） criteria.55 patients were classified to 4 grades：excellent（22.9%）,good（45.1%）,fair（26.9%）,poor（5.1%）.In 55 patients,excellent and good cosmetic results were found in 69.5% patients with sequential treatment and 68.4% in concurrent treatment group.There was no statistical significance（P0.05）.Conclu-sions For early stage breast cancer patients,the breast conserving treatment can get safe results and good cos-metic results.The sequencing of radiation and chemotherapy has not significant influence on cosmetic results.
