脑电图对额叶癫痫患儿诊断及治疗的指导价值

目的探讨脑电图对额叶癫痫患儿诊断及治疗的指导价值。方法选择2017年1月至2019年5月于我院确诊的80例癫痫患儿进行回顾性分析,对患儿实施磁共振成像(MRI)、常规脑电图、长程视频脑电图检查。比较三种检查方法的诊断结果;采用长程视频脑电图监测患儿治疗前、后的癫痫发作情况,评价其癫痫控制效果;并比较不同预后患儿的脑电图表现。结果长程视频脑电图检查对小儿额叶癫痫的诊断灵敏度、特异度、准确率均高于常规脑电图和MRI检查(P<0.05)。长程视频脑电图监测结果显示,治疗后,额叶癫痫患儿的癫痫发作次数明显少于治疗前,癫痫发作单次持续时间明显短于治疗前(P<0.05)。临床治疗总有效率为97.96%;预后良好组的局灶性痫性放电比例低于预后欠佳组,清醒期枕区节律性δ活动比例高于预后欠佳组(P<0.05)。结论长程视频脑电图对小儿额叶癫痫具有良好的诊断价值,还可对小儿额叶癫痫的治疗进行指导,对其治疗效果及预后进行评价。

Guiding value of electroencephalogram in the diagnosis and treatment of frontal lobe epilepsy in children

Objective To explore the guiding value of electroencephalogram in the diagnosis and treatment of frontal lobe epilepsy in children.Methods From January 2017 to May 2019,80 epilepsy children diagnosed in our hospital were selected for retrospective analysis.The children were examined by magnetic resonance imaging(MRI),routine electroencephalogram and long range-term video electroencephalogram.The diagnosis results of three kinds of examinations were compared;the epileptic seizures before and after treatment were monitored by long-term video electroencephalogram to evaluate the effect of epilepsy control;and the electroencephalogram manifestations of children with different prognosis were compared.Results The sensitivity,specificity and accuracy of long-term video electroencephalogram in the diagnosis of frontal lobe epilepsy in children were higher than those of routine electroencephalogram and MRI examination(P<0.05).The results of long-term video electroencephalogram monitoring showed that the number of epileptic seizures in children with frontal lobe epilepsy after treatment was significantly less than that before treatment,and the duration of epileptic seizures was significantly shorter than that before treatment(P<0.05).The total effective rate of clinical treatment was 97.96%;the proportion of epileptiform discharge in the group with good prognosis was lower than that in the group with poor prognosis,and the proportion of activity of rhythmicδin the occipital region during awake period was higher than that in the group with poor prognosis(P<0.05).Conclusion The long-term video electroencephalogram has a good diagnostic value for children with frontal lobe epilepsy,it can also guide the treatment of children's frontal lobe epilepsy,and evaluate its therapeutic effect and prognosis.
