宣白承气汤治疗痰热壅肺型老年重症肺炎的临床效果及对炎症因子的影响

目的探讨宣白承气汤治疗痰热壅肺型老年重症肺炎的临床效果及对炎症因子的影响。方法选择本院2018年1月至2018年12月收治的60例痰热壅肺型老年重症肺炎患者为研究对象,按随机双盲法将其分为对照组与治疗组,各30例,其中对照组给予西医常规治疗,治疗组在对照组的基础上加用宣白承气汤治疗。比较两组的临床疗效、中医证候积分、症状体征消失时间及炎症因子水平。结果治疗组的治疗总有效率显著高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的中医证候积分均下降,且治疗组低于对照组,差异具有统计学意义(P<0.05)。治疗组的咳嗽咳痰、发热及湿啰音消失时间均显著短于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的血清hs-CRP、IL-6、TNF-α水平均降低,且治疗组低于对照组,差异具有统计学意义(P<0.05)。结论宣白承气汤治疗痰热壅肺型老年重症肺炎可明显缓解患者的临床症状,改善炎症反应,从而有效提升临床疗效,值得推广应用。

Clinical effect of Xuanbai Chengqi decoction in the treatment of senile severe pneumonia with phlegm-heating and stagnation of lung type and its influence on inflammatory factors

Objective To investigate the clinical effect of Xuanbai Chengqi decoction in the treatment of senile severe pneumonia with phlegm-heating and stagnation of lung type and its influence on inflammatory factors.Methods From January 2018 to December 2018,60 elderly patients with severe pneumonia with phlegm-heating and stagnation of lung type admitted in our hospital were selected as the study objects.According to the random double-blind method,the patients were divided into control group and treatment group,with 30 cases in each group.The control group was given conventional western medicine treatment,and the treatment group was treated with Xuanbai Chengqi decoction on the basis of the control group.The clinical efficacy,TCM syndrome score,disappearance times of symptoms and signs,levels of flammatory factors of the two groups were compared.Results The total effective rate of treatment in the treatment group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).After treatment,the TCM syndrome score of the two groups decreased,and that in the treatment group was lower than the control group,the differences were statistically significant(P<0.05).The disappearance times of cough,expectoration,fever and rales in the treatment group were significantly shorter than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of serum hs-CRP,IL-6 and TNF-αin the two groups decreased,and those in the treatment group were lower than the control group,the differences were statistically significant(P<0.05).Conclusion Xuanbai Chengqi decoction in the treatment of senile severe pneumonia with phlegm-heating and stagnation of lung type can significantly alleviate the clinical symptoms of patients and improve the inflammatory response,thereby effectively improving the clinical efficacy,which is worthy of promotion and application.
