动态心电图在冠心病无症状性心肌缺血诊断中的应用价值

目的探讨动态心电图在冠心病无症状性心肌缺血诊断中的应用价值。方法选择2018年1月至2019年5月诊断为冠心病的124例患者作为研究对象,根据临床症状的的不同将其分为对照组(n=62,有症状性心肌缺血)和观察组(n=62,无症状性心肌缺血)。两组均使用相同的检测方法。比较两组的检测结果。结果观察组患者的平均心率低于对照组,心律失常发生率高于对照组(P<0.05)。观察组的全部窦性心搏RR间期、RR间期平均值标准差、相邻RR间期差值的均方根、相邻NN之差>50 ms的个数占总窦性心搏个数的百分比指标均低于对照组(P<0.05)。观察组的心肌缺血发作、持续时间长于对照组,Ⅳ大于对照组(P<0.05)。观察组心电图异常情况少于对照组,ST段阵次变化率高于对照组(P<0.05)。结论动态心电图用于诊断冠心病无症状性心肌缺血,能够明确患者心率以及心肌缺血的情况,具有极高的诊断价值,值得推广应用。

Application value of dynamic electrocardiogram in diagnosis of asymptomatic myocardial ischemia of coronary heart disease

Objective To investigate the application value of dynamic electrocardiogram in diagnosis of asymptomatic myocardial ischemia of coronary heart disease.Methods A total of 124 patients who were to be diagnosed with coronary heart disease from January 2018 to May 2019 were selected as study objects and divided into control group(n=62,symptomatic myocardial ischemia)and observation group(n=62,asymptomatic myocardial ischemia)according to the different clinical symptoms.Both groups used the same test method.The test result was compared between the two groups.Results The average heart rate of the observation group was lower than that of the control group,and the incidence of arrhythmia was higher than that of the control group(P<0.05).The all sinus cardiac RR interphase,standard deviation average of RR interphase,adjacent RR interphase root mean square,and proportion of adjacent NN difference>50 ms in the total number of sinus heart beat in the observation group were lower than those in the control group(P<0.05).The time and duration of myocardial ischemia attack in the observation group were longer than those in the control group,and IV was larger than that in the control group(P<0.05).Ecg abnormalities in the observation group were less than those in the control group(P<0.05).Conclusion The dynamic electrocardiogram for the diagnosis of asymptomatic myocardial ischemia of coronary heart disease can clarify the heart rate and myocardial ischemia of patients.It has a very high diagnostic value,which is worthy of popularization and application.
