内固定手术与关节置换术治疗老年股骨粗隆间骨折的临床效果

目的比较内固定手术与关节置换术治疗老年股骨粗隆间骨折的临床效果。方法回顾性分析我院骨外科收治的60例老年股骨粗隆间骨折患者的临床资料,根据治疗方法将其分为A组(30例,内固定手术)和B组(30例,关节置换术)。比较两组的治疗效果。结果B组的手术时间、术后引流时间、卧床时间、住院时间均长于A组,术中出血量多于A组(P<0.05)。术后3个月,B组的髋关节功能优良率高于A组(P<0.05)。术后1、2、3个月,两组的髋关节功能评分均高于术前,且B组均明显高于A组(P<0.05)。结论关节置换术可更好地改善老年股骨粗隆间骨折患者的髋关节功能,但其手术创伤更大。

Clinical effect of internal fixation and arthroplasty in the treatment of intertrochanteric fracture in elderly patients

Objective To compare the clinical effect of internal fixation and arthroplasty in the treatment of intertrochanteric fracture in elderly patients.Methods The clinical data of 60 elderly patients with intertrochanteric fracture admitted in our hospital were analyzed retrospectively.According to the treatment methods,the patients were divided into group A(30 cases,internal fixation)and group B(30 cases,arthroplasty).The therapeutic effects of the two groups were compared.Results The operation time,postoperative drainage time,bed rest time and hospitalization time in the group B were longer than those in the group A,and the intraoperative blood loss was more than that in the group A(P<0.05).Three months after operation,the excellent and good rate of hip joint function in the group B was higher than that in the group A(P<0.05).At 1,2 and 3 months after operation,the scores of hip joint function in both groups were higher than those before operation,and those in the group B were significantly higher than the group A(P<0.05).Conclusion Arthroplasty can better improve the hip joint function of the elderly patients with intertrochanteric fracture,but which has more traumatic.
