小儿大叶性肺炎的病原菌分布与肺功能分级的相关性分析

摘    要:目的分析小儿大叶性肺炎的病原菌分布与肺功能分级的相关性。方法选择我院收治的80例大叶性肺炎患儿,对所有患儿的肺部病原菌进行分离鉴定,并对病原菌分布与肺功能分级的相关性进行分析。结果革兰氏阳性菌检出率为18.75%,革兰氏阴性菌检出率为81.25%。随着肺功能等级的增高,革兰氏阴性菌检出率增高,而革兰氏阳性菌检出率降低(P<0.05)。金黄色葡萄球菌对ERY、CFZ、CRO耐药性分别为66.67%、75.00%、83.33%,鲍曼不动杆菌对AMP、ERY、CFZ耐药性分别为86.36%、68.18%、95.45%;大肠杆菌、肺炎克雷伯菌以及铜绿假单胞菌对AMP、GEN的耐药性≥45%。肺功能3、4级患儿的SP-A与AP1水平均显著低于1、2级患儿,且肺功能4级患儿显著低于3级患儿(P<0.05)。结论大叶性肺炎患儿的肺功能等级与病原菌分布以及血清SP-A与AP1水平具有一定的相关性。

Correlation analysis of pathogenic bacteria distribution and pulmonary function classification in children with lobar pneumonia

Abstract:Objective To analyze the correlation of pathogenic bacteria distribution and pulmonary function classification in children with lobar pneumonia.Methods A total of 80 children with lobar pneumonia admitted in our hospital were selected.Pulmonary pathogens were isolated and identified in all children,and the correlation between the pathogens distribution and pulmonary function classification was analyzed.Results The detection rate of Gram-positive bacteria was 18.75%,and the Gram-negative bacteria was 81.25%.With the increase of pulmonary function grade,the detection rate of Gram-negative bacteria increased,while the Gram-positive bacteria decreased(P<0.05).The drug resistance of Staphylococcus aureus to ERY,CFZ and CRO were 66.67%,75.00%and 83.33%,respectively,the Acinetobacter to AMP,ERY and CFZ were 86.36%,68.18%and 95.45%,respectively;and the E.coli,Klebsiella pneumoniae and Pseudomonas aeruginosa to AMP and GEN were≥45%.The levels of SP-A and AP1 in children with grade 3 and 4 of pulmonary function were significantly lower than those in children with grade 1 and 2,the levels of SP-A and AP1 in children with grade 4 of pulmonary function were significantly lower than those in children with grade 3(P<0.05).Conclusion The pulmonary function grade of the children with lobar pneumonia,the distribution of the pathogenic bacteria and the levels of serum SP-A and AP1 have a certain correlation.
