痔上黏膜环切吻合术与超声引导下痔动脉结扎术治疗混合痔的效果比较

目的比较痔上黏膜环切吻合术(PPH)与超声引导下痔动脉结扎术(DGHAL)治疗混合痔的效果。方法将我院肛肠科收治的96例混合痔患者根据手术方式分为PPH组和DGHAL组,各48例,比较两组的治疗效果。结果 DGHAL组的手术时间、创面愈合时间短于PPH组,术后出血率及VAS评分低于PPH组(P<0.05)。术后1、3个月,DGHAL组的肛管静息压、最大收缩压、直肠耐受量及直肠感知阈值量均优于PPH组(P<0.05)。术后1个月,两组的PF、RP、BP、SF评分均升高,且DGHAL组高于PPH组(P<0.05)。结论 PPH与DGHAL治疗混合痔短期内均有效,但DGHAL手术时间短、术后疼痛较轻,对肛管直肠动力学影响较小,且可提高患者生活质量。

Comparison of the effects of procedure for prolapse and hemorrhoids and Doppler guided hemorrhoidal artery ligation in the treatment of mixed hemorrhoids

Objective To compare the effects of procedure for prolapse and hemorrhoids(PPH) and Doppler guided hemorrhoidal artery ligation(DGHAL) in the treatment of mixed hemorrhoids. Methods A total of 96 patients with mixed hemorrhoids admitted in anorectal department of our hospital were divided into PPH group and DGHAL group according to the operation method, with 48 cases in each group. The treatment effects of the two groups were compared. Results The operation time and wound healing time in the DGHAL group were shorter than those in the PPH group, and the postoperative bleeding rate and VAS score were lower than those in the PPH group(P<0.05). At 1 and 3 month after surgery, the anal canal resting pressure, maximum systolic pressure, rectal tolerance and rectal perception threshold in the DGHAL group were better than those in the PPH group(P<0.05). One month after operation, the PF, RP, BP and SF scores of the two groups increased, and those in the DGHAL group were higher than the PPH group(P <0.05). Conclusion PPH and DGHAL are effective in the treatment of mixed hemorrhoids in a short period of time, but DGHAL has short operation time, less postoperative pain, less impact on anorectal dynamics, and can improve the quality of life of patients.
