PFNA与PFN治疗老年股骨粗隆间骨折的效果

目的分析股骨近端防旋髓内钉(PFNA)与股骨近端髓内钉(PFN)治疗老年股骨粗隆间骨折的效果。方法选择2017年11月至2018年12月我院收治的89例老年股骨粗隆间骨折患者为研究对象,以不同手术方式将其分为PFNA组(45例)以及PFN组(44例)。PFNA组患者接受PFNA治疗,PFN组患者接受PFN治疗。分析两组术后愈合时间、术中出血量、手术时间、并发症发生情况及髋关节功能。结果两组患者术后愈合时间及术中出血量比较,差异无统计学意义(P>0.05);PFNA组手术时间明显短于PFN组,差异具有统计学意义(P<0.05)。PFNA组的并发症总发生率为8.89%,显著低于PFN组的29.55%,差异具有统计学意义(P<0.05)。两组髋关节功能优良率比较,差异无统计学意义(P>0.05)。结论老年股骨粗隆间骨折患者采用PFNA治疗能取得满意效果,可以降低患者手术时间,促进疾病转归,具有推广价值。

Effect of PFNA and PFN in the treatment of intertrochanteric fractures in the elderly

Objective To analyze the effect of proximal femoral nail antirotation(PFNA)and proximal femoral nail(PFN)in the treatment of intertrochanteric fractures in the elderly.Methods Eighty-nine patients with intertrochanteric fractures of the elderly admitted in our hospital from November 2017 to December 2018 were selected as the study objects.The patients were divided into PFNA group(45 cases)and PFN group(44 cases)by different surgery methods.Patients in the PFNA group were treated with PFNA,and patients in the PFN group were treated with PFN.The postoperative healing time,intraoperative hemorrhage,operative time,complications and hip function of the two groups were analyzed.Results There were no significant differences in postoperative healing time and intraoperative hemorrhage between the two groups(P>0.05);the operation time of the PFNA group was significantly shorter than that of the PFN group,and the difference was statistically significant(P<0.05).The total incidence of complications in the PFNA group was 8.89%,which was significantly lower than 29.55%in the PFN group,and the difference was statistically significant(P<0.05).There was no significant difference in the excellent and good rates of hip function between the two groups(P>0.05).Conclusion PFNA in the treatment of the elderly patients with intertrochanteric fracture can achieve satisfactory results,reduce the operation time and promote the outcome of the disease,which has the value of promotion.
