256层螺旋3D-CT血管造影与DSA对颅内动脉瘤诊断的对比研究

摘    要:目的探讨256层螺旋3D-CT血管造影(3D-CT angiography,3D-CTA)与数字减影血管造影(digital subtractionangiography,DSA)对颅内动脉瘤的诊断价值。方法收集24例怀疑动脉瘤的病人,同时行256层螺旋3D-CTA及DSA检查,以DSA结果为金标准,评估3D-CTA检查的灵敏度,并比较两种影像学诊断方法在瘤体最长径测量上的差异。结果 3D-CTA发现动脉瘤的灵敏度为92.6%,两种影像学诊断方法在瘤体最长径测量上差异无统计学意义(P>0.05)。结论 256层螺旋3D-CTA对颅内动脉瘤的诊断灵敏度高,且能提供精确的信息和优质图像,但仍有部分漏诊。将CTA与DSA技术结合能更全面准确的诊断和观察颅内动脉瘤。

Abstract:Objective To investigate diagnostic value of 256-slice spiral three-dimensional CT angiography (3D-CTA) and digital subtraction angiography (DSA) in intracranial aneurysms. Methods Twenty-four patients with respected aneurysms were collected and examined by 256-slice spiral 3D-CTA and DSA. With DSA results as gold standard, sensitivity of 3D-CTA was estimated and the difference in measurement of longest diameter between the above two imaging examinations was compared. Results The sensitivity of 3D-CTA was 92.6% in the detection of aneurysms. There was no statistical significance in measurement of longest diameter between 256 slice 3D-CTA and DSA (P 〉 0.05). Conclusions 256-slice spiral 3D-CTA has a high sensitivity in diagnosis, which can provide accurate information and high-quality images, but some aneurysms are still missed. The intracranial aneurysms can be more comprehensively and accurately diagnosed and observed by the combination of CTA and DSA.
