眼眶特发性炎性假瘤的临床疗效分析

目的：探讨各种治疗眼眶特发性炎性假瘤(IOIP)的疗效，特别是手术与激素治疗的疗效比较。方法：对我院1978年1月到1999年12月住院的209例IOIP作系统的回顾性研究，分析各种方法的疗效并比较手术与激素两种方法对IOIP的疗效差别。结果：经手术、激素、局部放射、免疫抑制剂和COP方案化疗及上述方法联合治疗等(随访1个月到21年，平均1．5年)，治愈率40．2％(84/209)，部分治愈率57．4％(120/209)，复发率43％(34/80)。手术治疗与激素治疗比较表明手术治疗和手术联合激素治疗的效果优于单纯激素治疗的效果(P＜0．001)，但手术、激素及手术联合激素治疗三组间复发率无显著性差别(P＝0．573)。结论：手术、激素和局部放射等治疗IOIP有效率和复发率均高，手术和手术联合激素治疗的效果优于单纯激素治疗，特别提出手术治疗在IOIP中的作用。如何提高完全治愈率和减少复发率是今后努力的方向。

Therapeutic analysis of idiopathic orbital inflammatory pseudotumor

Objective:To study the therapeutic effects of surgery,corticosteriods and local radiation on patients with idiopathic orbital inflammatory pseudotumor (IOIP). Methods:209 cases of IOIP were reviewed with special attention to the therapeutic evaluation and the comparison of surgical management with corticosteroids therapy.The follow up time ranged from 1 month to 21years (mean 1 5 year). Results:The cure rate was 40 2% (84/209) and the partial cure rate 57 4% after different management.However the rate of recurrence was as high as 43 0%.Statistic analysios revealed that the therapeutic effects of surgery and surgery plus systemic steroid were superior to that of systemic steroid only (P<0 001).The rate of recurrence of IOPT did not demonstrate significant difference among the groups of surgery,systemic steroid and the combined surgery with systemic steroid (P=0 573). Conclusions:Both the effective and recurrence rates of treatment in partients with IOIP are high.The therapeutic effects of surgery and surgery plus systemic steroid are superior to that of systemic steroid only.The role of surgery in the management of IOIP should be emphasized.
