摘    要:目的探讨急性心肌梗死患者血尿素氮变化机制及对预后的影响。方法对2001年1月-2007年10月收治的AMI患者168例做动态BUN检测,并同步观察患者临床症状体征和有关实验室检查,对比分析BUN正常和BUN升高患者的年龄、性别、合并症、梗死部位、治疗方法及预后情况。结果BUN升高发生率为30.36%,病死率为25.49%;而BUN正常病死率仅为12.82%,差异有显著性(P〈0.05)。BUN升高组患者年龄,伴发高血压、糖尿病、动脉硬化和出现严重并发症情况,均与BUN正常组存在显著差异(P〈0.05),而且患者年龄越大,血压越高,BUN异常的发生率亦越高。结论BUN水平升高与患者年龄、血压、合并症等密切相关,且出现严重并发症和病死率远高于没有BUN异常的患者,提示有BUN异常的患者预后不佳。

Abstract:Objective To explore the mechanism and effect of prognosis of blood urea nitrogen(BUN)increase in patients with acute myocardial infarction(AMI).Methods 168 patients with AMI were selected,and the BUN and clinical date were analyzed. The age,sex,concomitant diseases,infarction region,treatment and prognosis were compared between patients with increase BUN and normal BUN level. Results The rate of BUN increase was 30.36%(51/168). The mortality of patients with increase BUN was 25.49%,which was higher than that of patients with normal BUN(12.82%)(P<0.05).There was significant difference in age,concomitant diseases (hypertension,diabetes,arteriosclerosis),serious complications between patients with increase BUN and normal BUN (P<0.05).The higher age and blood pressure,the higher rate of BUN increase.Conclusion The BUN increase was associated with age,blood pressure and concomitant diseases. And the mortality and rate of serious complications of patients with increase BUN was higher than that of patients with normal BUN . The BUN increase showed poor prognosis in patients with AMI.
