强直性脊柱炎全髋关节置换术的临床治疗分析

目的探讨人工全髋关节置换术治疗强直性脊柱炎的临床效果。方法选取2004年7月至2009年2月收治的行人工全髋关节置换术的强直性脊柱炎患者24例（33个髋关节）,对其临床资料进行回顾性分析。结果本组术后平均随访24个月,患者Merle D＇Aubigne评分：15～18分,平均17.2分;髋关节活动范围：屈伸平均50°～125°,外展内旋10°～50°,内外旋20°～45°。术后无切口及关节内感染,无脱位,髋关节疼痛消失。X线平片示假体位置良好,无松动,未出现异位骨化。术后髋关节活动度增加,屈曲挛缩畸形基本矫正（0°～5°）。结论人工全髋关节置换术可明显增加髋关节活动度,矫正髋关节畸形,改善髋关节功能,提高患者生活质量,是治疗强直性脊柱炎髋关节的有效方法。

Clinical Analysis of Ankylosing Spondylitis Total Hip Arthroplasty

Objective To evaluate the total hip arthroplasty in the treatment of ankylosing spondylitis clinical effects.Methods July 2004～February 2009 admitted and done total hip arthroplasty in patients with ankylosing spondylitis in 24 patients（33 hips）,the clinical data were retrospectively analyzed.Results The mean follow-up of 24 months,the patient Merle D＇Aubigne score： 15～18 points,an average of 17.2 points;hip range of motion： flexion and an average of 50°～125°,outreach internal rotation 10°～50°,internal and external rotation 20°～45°.No postoperative incision and intra-articular infection,no dislocation,hip joint pain disappeared.x-ray showing implant in good position,no loosening,heterotopic ossification does not appear.Increase in activity after hip flexion contracture deformity of the basic correction（0 ° ～ 5 °）,life returned to normal.Conclusion Total hip arthroplasty can significantly increase hip mobility,correction of hip deformity,to improve the hip joint function,improve patient quality of life,the treatment of ankylosing spondylitis hip effective manner.
