三维适形放疗治疗宫颈癌复发患者60例临床观察

目的探讨三维适形放射治疗宫颈癌复发患者的临床疗效与不良反应。方法选取子宫颈癌治疗后复发患者60例,以直线加速器6mv-x线行三维适形放疗,单次剂量2~2.2Gy,每日1次,每周5次,总剂量56~60Gy,平均剂量58.5Gy。结果阴道出血缓解率100%,腰腿疼痛缓解率达100%,患侧下肢水肿缓解率67%,肿瘤治疗有效（CR＋PR）率为76.7%（46/60）,1和3年生存率分别为38%和13%,中位生存时间18个月。放射性肠炎近期反应发生率为8.3%;放射性膀胱炎近期反应发生率为3.3%,骨髓抑制（Ⅰ度）发生率为10%,放射性肠炎远期并发症发生率6.7%,放射性膀胱炎远期并发症发生率为5%。结论三维适形放疗效治疗复发性子宫颈癌疗效较好,放疗并发症少,不良反应小。

Clinical Research on Treatment of Recurrent Cervical Carcinoma with Three-dimensional Conformal Radiotherapy

Objective To explore the clinical effect of the treatment of recurrent cervical carcinoma with three-dimensional conformal radiotherapy（3D-CRT）.Methods Sixty patients with recurrent cervical carcinoma were irradiated by three-dimensional conformal radio-therapy.The regimen was：total dose 56~60Gy,2~2.2Gy/f,1f/d,5f/w.Results The rates of sciatic pain and leg edema relief response were 100% and 65% respectively.The CR＋PR response rate was 76.7%.The overall 1-and 3-year survival rates were 38% and 6%.Conclusion Three-dimensional conformal radiotherapy is much effective for recurrent cervical carcinoma,but the former had a low rate of complications and shortened treatment term.
