地诺前列酮栓与缩宫素注射液用于足月妊娠促宫颈成熟的临床观察比较

目的观察比较地诺前列酮栓（普贝生）和缩宫素注射液用于足月妊娠促宫颈成熟的效果,评价其有效性与安全性。方法选择2008年3月至2010年3月成都市锦江区妇幼保健院产科住院的86例孕妇随机分为对照组和普贝生组,每组43例。普贝生组孕妇阴道后穹隆放置普贝生1枚,对照组孕妇给予5%葡萄糖500mL加入1U缩宫素静脉滴注,对2组孕妇的Bishop评分、产后出血、临产情况、用药至临产发作时间、阴道分娩率、剖宫产率及新生儿窒息率进行比较。结果普贝生组总有效率和24 h内阴道分娩率均显著高于对照组（P〈0.01）,普贝生组用药至临产时间显著短于对照组（P〈0.01）,治疗组剖宫产率显著低于对照组（P〈0.01）。普贝生组的新生儿窒息率和产后出血率明具有降低的趋势,但在统计学上无显著差异（P〉0.05）。结论普贝生用于足月妊娠促宫颈成熟引产优于缩宫素注射液,是一种安全、有效并且使用方便的促宫颈成熟引产药物。

Clinical Observation of Dinoprostone Suppository and Oxytocin Injection to Cervical Ripening in Term Pregnancy

Objective To observe and compare the results of dinoprostone suppository and oxytocin injection to cervical ripening,and evaluate the efficiency and safety of dinoprostone suppository.Methods Totally 86 primiparas in hospital between January 2009 and March 2010 were selected to divide into two groups（control group and Propess group）.There were 86 cases in each group.One piece of Propess was put into the vaginal posterior fornix of gravidas in Propess group,and Oxytocin injection 1U added in 500 ml of 5% glucose transfusion was administrated by iv.in control group.The maternal cervical Bishop score,postpartum hemorrhage,condition of labor starting,the time from given medicine to labor starting,vaginal delivery rate,cesarean section rate and neonatal asphyxia rate were compared between two groups.Results The total effective rate and vaginal delivery rate in 24 hours in Propess group were significantly higher than those in control group（P0.01）;The time from given medicine to labor starting in Propess group was shorter than that of control group（P0.01）.The cesarean section rate in Propess group was significantly lower than that in control group（P0.01）.The postpartum hemorrhage and neonatal asphyxia rate in Propess group had the decreased tendency,but there was no significant difference in statistic between two groups（P0.05）.Conclusion Dinoprostone is superior to Oxytocin injection for cervical ripening and artificial labor in term pregnancy,and safe,effective and convenient medicine to treat cervical ripening and artificial labor.
