腔镜下经胸乳入路小切口甲状腺手术临床体会

目的探讨经胸乳入路腔镜下小切口甲状腺手术的方法与体会。方法对50例甲状腺良性病变患者采用胸乳入路小切口内镜手术,建立皮下空间,内镜下超声刀行甲状腺部分或次全切除术。结果 50例腔镜下顺利完成手术,手术时间50～140min,平均85min;术中出血量15～60ml,平均40ml;术后6h下床活动,进流质,术后2天拔除颈部引流管;术后住院3～5d,平均4d。50例术后随访3～24个月,平均12个月,均无肿瘤复发,乳晕切口无瘢痕形成,美容效果好。结论内镜下甲状腺手术美容效果好,手术并发症少安全性高,值得临床推广使用。

Endoscopy through Small Incision Thyroid Surgery

Objective To investigate the chest small incision into the endoscopic approach and experience of thyroid surgery.Methods Totally 50 cases of benign thyroid disease were treated with Chest small incision endoscopic surgery,to establish subcutaneous space,CO2 pressure 6mm Hg,endoscopic ultrasound scalpel part or subtotal thyroid resection.Results 50 cases of successful com-pletion of endoscopic surgery,operative time 50 ～140min,an average of 85min;blood loss was 15 ～ 60ml,an average of 40ml;6h after getting out of bed,into the liquid,2 days after removal of drainage tube;postoperative hospital stay 3 ～5d,the average 4d.50 cases were followed up for 3 to 24 months,an average of 12 months,no tumor recurrence,the areola incision scar formation,side effects.Conclu-sion Endoscopic thyroidectomy has good cosmetic results,less complications and safer,is worthy of clinical use.
