43例下肺野结核临床影像学特点分析

目的总结分析下肺野结核的临床及影像学特点，提高对该疾病的诊断水平。方法对43例均经病理、痰查结核菌或抗结核治疗愈合的下肺野结核病例的临床资料及常规X线胸部正侧位片、CT扫描资料进行回顾性分析。结果下肺野结核以中青年多见，发热、咳嗽、咳痰、咯血为其主要症状；其影像学表现多样：可以表现为大片实变为主、小叶片状渗出性病灶为主、空洞病灶为主及肿块病灶为主型改变，但常伴有增殖、纤维性病灶、“卫星”灶和同侧或对侧支气管播散病灶。CT检查较X线敏感，肿块型结核病灶增强扫描无强化或仅为环形强化。结论下肺野结核尽管临床症状资料不典型，但大部分病灶仍有其一定的影像学特征，仔细分析阅片，提高对本病的警惕性，注意鉴别诊断，可明显提高诊断的正确率。

Clinical and imaging analysis of 43 patients with pulmonary tuberculosis in lower fields

Objective To summarize and investigate the clinical and imaging characteristics of lower lung field tuberculosis and improve diagnostic accuracy of these diseases. Methods The clinical information and the X ray findings of 43 patients with pulmonary tuberculosis in lower fields were analyzed retrospectively. ResuIts Lower lung field tuberculosis were more seen in youth. The clinical symptoms including fever, cough, hemoptysis, etc. According to the X ray findings of pulmonary tuberculosis in lower field were categorized to four types： lobar patchy consolidation; small exudation shadow, cavitation lesion, and spherical shadow, which commonly associated with fibrotic and proliferative lesion, surrounding ＂satellite＂ lesions. Conclusion The clinical information of pulmonary tuberculosis in lower field is not typical, but most lesions of these diseases still have some imaging characteristic findings, To study its X-ray charcteristics seriously and pay attention to its occurence it can be accurately diagnosed.
