运用外后侧入路治疗胫骨平台骨折

目的 通过对胫骨平台外后侧手术入路的讨论,验证其在治疗胫骨平台骨折中的指征、作用、局限性.方法 采用外后侧入路治疗患者28例,平均年龄34.7岁；骨折类型:均明显波及外后侧平台,按照Schatzker分型:14例为Ⅱ型,2例为Ⅲ型,8例为Ⅳ 型,4例为Ⅴ型.术后平均随访14.5个月.结果 Neer评分表评价膝关节功能,优21例,良6例,差1例；随访中发现3个月后骨折临床愈合,可弃拐负重行走.出现1例关节伸直轻度粘连,2例腓总神经轻度损伤.结论外后侧胫骨平台骨折治疗中,外后侧入路是一个疗效肯定、风险较小的选择.

Lateral-posterior approach for tibial plateau fracture

Objective To report the technique,indication and results of ORIF using lateral-posterior approach in the treatment of tibial plateau fracture.Methods A retrospective analysis of 28 consecutive cases of tibial plateau fracture was conducted.Their average follow-up was 14.5 months long.Their average age was 34.7 years old.According to Schatzker’s classification,14 cases were type II,2 were type III,4 were type IV,8 were type V.Results According to Neer’s scale,the knee joint function was evaluated as excellent in 21 patients,good in 6 cases,bad in one case.3 months after operation,the fractures were all evaluated as clinically healing,ambulation without crutch was permitted.Joint adhesion was found in one case,minor nerve injury was found in 2 cases.Conclusion ORIF using lateral-posterior approach is the credible method for the treatment of tibial plateau fracture.
