双镜联合保胆取石术与完全腹腔镜保胆取石术治疗胆囊结石的临床研究

目的对比研究腹腔镜辅助胆管镜保胆取石术与完全腹腔镜保胆取石术,探讨其临床可行性。方法 2007年3月～2009年10月行腹腔镜辅助胆管镜保胆取石术(观察组)53例,完全腹腔镜保胆取石术(对照组)49例,对比分析两组手术时间、出血量、术后肠蠕动恢复及并发症。结果 102例患者均完成手术,观察组平均手术时间少于对照组(P<0.05)。两组手术出血量及术后肠蠕动恢复差异均无统计学意义(P>0.05)。两组术后均无并发症发生。结论腹腔镜辅助胆管镜保胆取石术安全可行,但两种术式的远期疗效尚待深入研究和对比观察。

Clinical comparative study of laparoscopic-assisted gallbladder-preserving cholelithotomy and totally laparoscopic gallbladder-preserving cholelithotomy

Objective To investigate the feasibility of laparoscopic-assisted gallbladder-preserving cholelithotomy(LAGPC).Methods The clinical data of 53 patients with LAGPC(observed group) and 49 patients with totally laparoscopic gallbladder-preserving cholelithotomy(TLGPC)(controlled group) between Mar 2007 and Oct 2009 were analyzed.The operation time,bleeding volume,enterokinesia recovery time and complication were compared between two groups.Results All operations were successful and stone were clear.The operation time in observed group was significantly less than that in controlled group(P<0.05).And the bleeding volume and enterokinesia recovery time between two groups were not statistically signi-cant(P>0.05).No complication occurred.Conclusion LAGPC is a safe and feasible procedure.However,the long-term effects of two procedures need further analysis and comparative study.
