ERCP在急性胆源性胰腺炎的应用

目的 探讨ERCP在治疗急性胆源性胰腺炎(ABP)中的临床应用价值.方法 对57例经ERCP治疗(观察组)和41例非ERCP治疗(对照组)的ABP患者的临床资料进行回顾性分析,观察组27例行急诊ERCP治疗,30例行非急诊ERCP治疗.结果 观察组的腹痛缓解时间、血淀粉酶恢复正常时间、住院天数、住院费用、并发症发生率及复发率均显著低于对照组(P均＜0.05).观察组轻症患者中急诊行ERCP治疗的腹痛缓解时间、血淀粉酶恢复正常时间、住院天数和住院费用与非急诊ERCP治疗比较差异无统计学意义,P均＞0.05.重症急诊患者的腹痛缓解时间、血淀粉酶恢复正常时间显著低于非急诊者,而住院天数和住院费用比较差异无统计学意义.结论 ERCP治疗ABP安全且疗效显著,并可有效预防复发.

Application of ERCP in patient with acute biliary pancreatitis

Objective To investigate value of clinical applications of ERCP in patient with acute biliary pancreatitis.Methods 98 cases of patients with ABP were randomly divided into observation group(n=57) and control group(n=41) groups,were respectively treated with ERCP and internal medical therapy.27 cases and 30 cases of observation group were respectively treated with acute ERCP and no acute ERCP.Results The observation group in hospitalization days,time to ease abdominal pain,serum amylase returned to normal time,the incidence of complications,hospitalization costs and mortality rate were lower than the control group,the statistically significant differences(P<0.05).The non serious paeients with acute ERCP of observation group in hospitalization days,time to ease abdominal pain and serum amylase returned to normal time and hospitalization costs were no statistically significant differences compared with the non acute patients(P>0.05).The serious paeients with acute ERCP of observation group in time to ease abdominal pain and serum amylase returned to normal time were lower than the non acute patients,but hospitalization days and hospitalization costs were no statistically significant differences.Conclusion ERCP is security and significantly effective for ABP,and effectively prevent recurrence.
