摘    要:目的研究C-肽与2型糖尿病(T2DM)肾脏病变(DN)、视网膜病变(DR)之间的关系。方法选择T2DM病人403例,根据有无DN、DR、DPN,并按糖尿病病程≤5年、〉5-≤10年,〉10-≤15年,〉15年进行分组,比较不同病程中各组C-肽的水平,用Logistic多元回归分析微血管病变的危险因素。结果不同病程中,有DN、DR的病人血清C-肽分别低于无DN、DR的病人,病情越重,C-肽下降越明显,P〈0.05或P〈0.01;Logistic回归分析,FCP是DN、DR的保护因素,P均为0.000。结论血清C-肽是DN、DR的保护因素,其下降可能导致了T2DM微血管病变的发生发展。

Relationship between C-peptide and Type-2 Diabetes Microangopathy in Different Durations

Abstract:Objective To study the relationship between serum C - peptide level and type -2 diabetes nephropathy(DN) and diabetic retinopathy(DR). Methods 403 type -2 diabetic patients were included in the study. According to diabetic duration≤5 years, 〉5 -≤10 years, 〉 10 -≤15 years, 〉 15 years, the patients were classified based on presence or absence of DN, DR or DPN. The level of serum C - peptide of every group was compared in defferent duration. The risk factors of microangopathy were determined with the logistic analysis. Results In differents durations, the level of FCP in type 2 diabetes with DN or DR was lower than in the patients without DN or DR respectively, the more serious of the state, the lower of the level of serum C -peptide is, P 〈 0.05 or P 〈 0. 01; Logistic regression analysis showed that FCP was a protective factor of DN or DR, P = 0.000 respectively. Conclusion Serum C - peptide was a protective factor of DN or DR. The decrease of serum C - peptide probably leads to the development of microvascular complications in T2DM.
