常温下第一肝门血流间歇阻断行肝叶切除的体会

常温下第一肝门间歇阻断切肝法操作简单,出血少,易于推广。本文就14例肝癌病人术中切口选择、肝脏的暴露以及较易损伤的血管(肝静脉、肝短静脉、下腔静脉)出血的预防和处理进行了讨论,认为术中对肝脏进行“双合诊”是估计病肝是否能切除和预防术中大出血的有效措施。

THE EXPERIENCE OF LIVER RESECTION IN NORMOTHERMIC FIRST HEPATIC PORTAL INTERMITTENT ISCHEMIA

The method of liver resection in normothermic first hepatic intermittent ischemia is technically simple, less bleeding and easy to extent. This paper reports 14 cases suffering from primary liver carcinoma that the hepatic lobectomy was performed. For the choice of incision, expossure of liver, prevention and management of bleeding from the vessel that easy to injure (hepatic vein. short hepatic vein, inferior vena cava) are discussed. The authors consider that the bimanual palpation of liver in operating is an effective measure for the expectation of hepatic operability and prevention of massive bleeding.
