胃癌中表皮生长因子受体2蛋白的表达及其临床意义

目的：探讨表皮生长因子受体2（HER2）蛋白在胃癌及癌旁胃组织中的表达，分析其与胃癌临床病理特征的关系。方法用免疫组织化学EnVision法检测579例胃癌组织和65例癌旁胃组织中HER2蛋白表达并进行比较。结果胃癌和癌旁胃组织中HER2过表达率分别为10.54%（61/579）和0（0/65），两者HER2过表达率差异有统计学意义（P＜0.01）。肠型胃癌HER2蛋白过表达率（17.90%）明显高于弥漫型胃癌（4.18%）和混合型胃癌（8.57%），均P＜0.01；高-中分化胃癌HER2过表达率（14.91%）明显高于低分化胃癌（6.58%），P＜0.01。HER2蛋白表达与胃癌临床分期、淋巴结转移及浆膜浸润无关（均P＞0.05）。结论胃癌中HER2表达增强，其高表达与肠型胃癌和高-中分化胃癌密切相关。肠型胃癌和高-中分化胃癌中HER2高表达的机制有待进一步探索。

Expression of HER2 in gastric carcinoma and its clinicopathological significance

Objective To investigate the expression of epidermal growth factor receptor 2（HER2） protein in gastric carci-noma and its clinicopathological significance. Methods The expression of HER2 was detected with immunohistochemical EnVi-sion method in 579 specimens of gastric carcinoma and 65 specimens of adjacent gastric tissue. Results The positive expres-sion rates of HER2 protein in gastric carcinomas and adjacent gastric tissues were 10.54%（61/579） and 0（0/65）, respectively（P〈0.01）. The HER2 expression rate in intestinal type gastric carcinomas （17.90%） was significantly higher than that in diffuse type gastric carcinomas（4.18%） and mixed type gastric carcinomas（8.57%）（P〈0.01）. The expression rate of HER2 in wel and moder-ately- differentiated gastric carcinomas was significantly higher （14.91%） than that in poorly differentiated ones （6.58%）（P〈0.01）. No association was observed between HER2 expression and clinical stage, lymph nodes metastasis and serosal invasion （P〈0.05）. Conclusion HER2 expression is upregulated in gastric carcinoma, particularly in intestinal type gastric carcinomas and wel and moderatelydifferentiated gastric carcinomas.
