宫颈原位癌中Ki-67的表达及其与细胞凋亡的关系

摘    要:目的 探讨宫颈原位癌中Ki 6 7的表达及其与细胞凋亡的关系 ,及对原位癌生物学特性的影响。方法 采用免疫组化SP法检测石蜡包埋宫颈组织中Ki 6 7的表达 ,包括研究组宫颈鳞状上皮原位癌 30例 ,对照组正常上皮组织 10例、中重度不典型增生 2 0例和浸润癌 15例。并用TUNEL法原位检测细胞凋亡 ,以 10 0 0个细胞中增生细胞和凋亡细胞分别作为增生指数PI(‰ )和凋亡指数AI(‰ )。结果 PI在宫颈原位癌中明显高于正常组和不典型增生组 ,但又显著低于浸润癌组 (P <0 0 1) ,AI在宫颈原位癌中明显高于正常 ,但又显著低于浸润癌 (P <0 0 5 ) ,且PI和AI呈显著正相关 (P <0 0 1) :4组中PI的变异性小 ,AI的变异性大 ,尤其是原位癌组。结论 宫颈原位癌是癌变过程中一个相对特殊阶段的癌 ,癌细胞的增殖活性虽然明显高于正常组和不典型组 ,但其增殖和浸润能力仍受到一定限制。细胞凋亡变异对原位癌的发展可能影响更大

Abstract:Purpose To investigate the relationship between the expression of Ki-67 and the apoptosis, and the effect of them on the biological characteristics of carcinoma in situ of cervix. Methods Seventy-five cases of cervical samples comparising 30 cases of carcinoma in situ (SCIS), 10 cases of normal squamous epithelium (NSE), 20 cases of dysplasia (DSEH) and 15 cases of invasive carcinoma (ISEC) were studied. The proliferating cells were detected by immunohistochemical staining (SP method) with Ki-67 monoclonal antibody MIB1.The apoptotic cells were detected in situ by the method of TDT mediated dUTP-biotin nick end-labeling (TUNEL). Results Expression of Ki-67 had an increasing trend in NSE, DSEH, SCIS and ISEC. The PI had significant difference between SCIS and DSEG, SCIS and ISEC respectively. There was no apoptotic cell in NSE. Expression of AI had an increasing trend in DSEH, SCIS and ISEC. The AI had significant difference between SCIS and NSE, SCIS and ISEC respectively. There was a statistically significant correlation between PI and AI. The variance of AI was bigger in SCIS than that of PI. Conclusion Carcinoma in situ is in a special stage in the oncogenesis.The proliferating activity of cervical carcinoma in situ which is higher than that of NSE and DSEH is limited in some degree.The apoptosis of cancer cells may have more important effects on the development of carcinoma in situ.
