舒张早期左心室流入血流色阶差的二维彩色多普勒超声研究

目的 应用二维彩色多普勒色阶差方法检测左室流入速度梯度，了解其在评价心脏病患者左室舒张功能异常中的实用性。方法 使用自制左室流入彩色多普勒图像分析系统，自动计算舒张早期沿左室流入彩色多普勒血流的色阶差数据，计算左室流入血流的平均压力阶差（IVPG）与彩色血流色阶奇异值（color singular value，CSV）。结果 心肌梗死、扩张型心肌病、冠心病各组的IVPG较对照组显著减低，CSV显著增高（P〈0．001）。结论 左室舒张早期二维血流色阶差方法能够较全面地反映舒张早期左室血流的速度梯度变化，客观体现左室整体舒张功能的变化。

Color differences of left ventricular inflow at early diastole: a two-dimensional color Doppler study

Objective A new method for analyzing color differences and pressure gradients within left ventricular inflow by two-dimensional color Doppler image was applied to evaluate the ability of this method in accessing left ventricular function in patients with left ventricular dysfunction.Methods The color Doppler data including intraventricular pressure gradients(IVPG) and color singular value(CSV) were collected with the two-dimensional color Doppler image analyzing system.Results Compared with data in control,IVPG in groups of myocardial infarction,dilated cardiomyopathy and coronary heart disease was decreased and CSV left ventricular inflow in these groups was increased significantly(P<(0.001)).Conclusions The method of color differences and pressure gradients analyzing in left ventricular inflow by two-dimensional color Doppler could reflect the changes of left ventricular pressure gradients at early diastole,which could describe the total left ventricular function objectively.
