小肠肿瘤87例

[背景 ]探讨小肠肿瘤的诊断和治疗 .[病例报告 ]87例小肠肿瘤的主要临床表现有消化道出血、腹痛、黄疸、腹部肿块、肠梗阻及肠穿孔等 ,其中良性肿瘤为 30例 ,恶性肿瘤为 5 7例 ,术前确诊率为6 1% ,其中十二指肠肿瘤均术前确诊 ,而空回肠肿瘤术前确诊率仅为 32 % .胃镜、十二指肠镜、十二指肠气钡双重造影、B型超声波及CT检查对十二指肠肿瘤的确诊率较高 ,小肠气钡双重造影、选择性肠系膜动脉造影检查对空回肠肿瘤的诊断率较高 .[讨论 ]胃十二指肠镜检查结合病理活检是诊断十二指肠肿瘤的首要手段 ,而小肠气钡双重造影、选择性肠系膜动脉造影检查则有助于空回肠肿瘤的早期诊断

87 cases of primary tumors of small intestine

BACKGROUND To explore the diagnosis and treatment for primary tumors in small intestine.?CASE REPORT?The data of 87 cases of primary tumors in the small intestine were retrospectively analyzed. The main manifestations of tumors in small intestine are gastrointestinal hemorrhage, abdominal pain, jaundice, abdominal mass, ileus and intestine perforation. Of 87 cases of tumors, 30 were benign and 57 were malignant. The rate of diagnosis before operation was 61%, all the duodenal tumors were diagnosed preoperatively, but only 32 % of jejunal and ileum tumors were diagnosed correctly. Gastroscopy,duodenoscopy,coloscopy,B type ultrasonic wave and CT scan are useful for the dectection of duodenal tumor, so are the enteroclysis and selective mesentery angiography for the tumors of jejunum and ileum.?DISCUSSION?As for the patients with chronic abdominal pain,gastrointestinal hemorrhage,abdominal masses,intestinal obstruction and jaundice, endoscopy B Ultrasound, CT should be first applied, if there are no positive findings, enteroclysis and selective mesentery angiography should be used for further examination.
