缝匠肌蒂骨瓣植骨治疗45例青壮年股骨颈骨折

为解决青壮年股骨颈骨折较易发生骨不愈合和股骨头缺血性坏死的难题，自１９８０年３月～１９９２年５月采用缝匠肌骨瓣嵌入植骨治疗青壮年股骨颈骨折４５例．其中４０例获得随访，随访时间最短２年，最长１２年，平均随访时间５年６个月．疗效评定：优２４例，良１２例，优良率９０％．骨折愈合３８例，愈合率９５％；骨愈合时间３～１２个月，平均５个月．２例骨不愈合，股骨头缺血性坏死；２例骨愈合后迟发性股骨头节段性塌陷．股骨头坏死率１０％．认为此术式是治疗青壮年股骨颈骨折较好的方法．

Treatment of Fracture of the Femoral Neck in Young Adults with Sartorial Pedicled Iliac Bone Grafts

In order to solve the problem of the fracture of the femoral neck in the young adults that becomes nonunion and avasular necrosis of femoral head easily,forty five cases that sustained the fracture of the femoral neck in young adults were treated with sartorial pedicled iliac bone grafts in our department from March 1980 to May 1992.Forty cases of the 45 were followed up for 2 to 12 years with an average of 5.6 years.The results were defined as excellent in 24 patients,good in 12,being as high as 90%.Union of the fracture of the femoral neck was seen in 38 cases (95%).The time for union of the fractures extended from 3 to 12 monthes with an average of 5 monthes.Nonunion and avascular necrosis of the femoral head was seen in 2 cases.Segmental collapse of the femoral head was seen in 2 cases after union.Necrotic rate of the femoral heads was 10%.The authors believe that this operation is a better way for treatment of the fracture of the femoral neck in young adults.(Original article on page 209)
