猪辅助性异位肝移植的实验研究

我院于1995年5月－12月进行猪辅助异位肝移植10次，供体取减体右肝，切除胆囊和左侧二叶肝。移植肝植于受体猪的右肝下，供肝的肝上下腔静脉与受体肝下下腔静脉端侧吻合，供肝的门静脉与受体门静脉端侧吻合。供肝肝动脉与受体主动脉端吻合。供肝胆总管与受体十二指肠吻合。10例手术均成功。术后生存2．5h－4天5例，另4例生存健康，2例在3个月处死，2例在6个月处死，证明移植肝存活。成功的体会是：1、血管吻合是辅助肝移植成功的关键；2、移植肝重量与受体体重比例最好是在1％左右；3、谷丙转氨酶、总胆汁酸、血氨和总胆固醇可作为实验动物的参考指标。

Auxiliry Heterotopic Liver Transplantation in Pigs

Ten cases of achlialy heterotopic live transplantation in pigs were performed from May to Dec. 1995 in our hospital. The donor reducing - graft was right liver with cholecystectomy and two left lateral lobes resected . The graft was placed in the right subhepatic space of the host . The following anastomoses were done: donor (D) suprahepatic inferior vena cava (IVC) end - to - side to host (H) infrahepatic IVC, D - PV end - to - side to H - PV, D - HA to Haorta and D - common bile duct to H - duedenum. All of transplantative operation were successful. The survival in 21 weeks were 6 cases, surviving animal of 4 cases with good health were killed on postoperative 3 months in 2 cases and 6 months in 2 cases. The experience of successful transplantation was that: 1. Vascular anastomoses was the key of successful tiansplant. 2. Weight of ratio between the graf and the host should be 1% or so. 3. ALT,bile - acids, blood - penia and blood cholesterol were good parameter for the prognosis of transplantation.
