高危人乳头瘤病毒、p1 6、p53蛋白在胃癌和癌前病变中的表达

目的探讨高危HPV感染与抑癌基因表达产物p53、p16蛋白在胃癌发生、发展中的作用及与肿瘤预后的关系.方法应用原位杂交和免疫组化技术,分析96例胃癌和癌前病变及腺癌组织中HPV-1/18DNA、P53和p16蛋白的阳性表达情况.结果HPV-16亚型和18亚型的阳性率分别为34.38%和8.33%;P53和p16蛋白阳性表达率分别为46.88%和43.75%.HPV-16亚型、P53和p16蛋白在癌前病变中的阳性表达率与腺癌组织有显著差异(P＜0.05),HPV-16和P16蛋白、P53蛋白之间具有显著相关性(P＜0.05).结论HPV-16亚型感染、P53及p16蛋白的水平表达与胃癌的呈正相关,p16蛋白的低表达率是肿瘤预后差的标志之一.

Detection of high risk human papillomavirus, p16 and p53 proteins in gastric carcinoma and precancerous lesions

Objective To investigate the roles of HPV-16/18 infection and p16 and p53 expression in the pathogenesis and development of gastric carcinoma and their prognostic value. Methods In situ hybridization and immunohistochemical staining were used to detect HPV-16/18 and p16 and p53 proteins in 96 cases of gastric carcinoma as well as precancerous lesions. Results Positive rates of HPV-16 and HPV-18 were 34% and 8% differently; Expression of p16 and p53 was seen in 47% and 44% cases of the tumor respectively. There was a significant difference ( P <0.05) in the expression of p16 and p53 between gastric carcinoma and the precancerous lesions. HPV-16 infection was closely related to aberrant expression of p16 and p53 ( P <0.05). Conclusions Infection of HPV-16 subtype and the expression of p16 and p53 in different levels are positively correlated with the development of gastric carcinoma. Decreased expression of p16 can be regarded as a poor prognostic marker for the patients.
