急性胰腺炎严重指数对临床预后的评估价值

目的：综合ＣＴ影像提出急性胰腺炎严重指数（ＡＰＳＩ）的评估方法并测定其敏感性，提高ＣＴ影像对急性胰腺炎预后评估的准确率。材料与方法：Ｑ发 胰腺炎患者６５例，以住院时间及禁食时间为应变量（Ｙ）、胰腺肿大、胰周和／或腹腔内渗出液及胰腺坏死情况作为自变量（Ｘ），用多元线性回归分析法对ＡＰＳＩ、ＣＴ严重（ＣＴＳＩ）及简化急症生理（ＳＡＰ）计分法进行比较。并分析此三者对并发症发生率和死亡率的预评价值。结果：

Predictive Value of APSI for Acute Pancreatitis Prognosis

Objective: Based on CT findings, acute pancreatitis severity index (APSI) was proposed in order to improve the accuracy and sensitivity of prognostic evaluation in acute pancreatitis. Materials and Methods: The hospitalization days and fasting days were used as dependent variables, CT findings, including pancreatic edema, fluid collection and pancreatic necrosis, were used as independent variables. Using multiple linear regression analysis, APSI was compared with CTSI and clinical SAP score. The predicting accuracy of APSI, CTSI and SAP for complications and mortality was evaluated. Results: Pancreatic edema and necrosis were highly correlated with hospitalization days and fasting days ( P <0.01 in both). APSI was most highly correlated with hospitalization and fasting days (WT5BXP <0.0001), and was more sensitive than CTSI in predicting of complications and mortality. Conclusion: APSI has positive correlation with clinical prognosis. The value of APSI for predicting AP prognosis is superior to CT
