| 摘 要： | 目研究了基于健康档案区域卫生信息平台的慢病管理技术路径，以及利用区域慢病业务平台对慢病患者提供区域一体化协同服务模式，实现区域健康管理业务联动，解决信息隔离、服务孤立等问题，为基于区域卫生信息平台的慢病管理模式提供了有益的探索。
