血管鞘在脑出血微创血肿引流术中的应用

摘    要:目的 介绍一种把血管鞘稍作改制作为引流装置在脑出血微创血肿引流术中应用的方法,并对其可行性进行评估。方法 2004年以来对头颅CT扫描诊断的12例基底节区脑出血患者以血管鞘作为引流装置实施血肿微创引流手术,观察手术的操作、引流效果、并发症。结果 用改制的血管鞘作为引流装置行基底节区脑出血血肿引流术,12例患者手术均成功,其中6例患者经过复查头颅CT调整了塑型的引流管头端增加了血肿引流量,术后扫描的CT图像无伪影。术后平均拔管时间为7.5d,血肿平均引流量为85%。手术患者死亡2例,1例死于吸入性窒息,1例死于肾功能衰竭,未发生与手术相关的并发症。结论 改制的血管鞘作为引流装置用于基底节区脑出血微创血肿引流术中可行性好,安全有效,且具有头端根据需要能塑型及带管引流中复查CT无伪影的特点。

Abstract:Objective To introduce a new technique of slightly improved vessel sheath in the operation of basilar ganglion cerebral hemorrhage due to invasive surgery. Methods The study was carried out in the Department of Neurology, the Second Hospital of Lanzhou University from March 2004. 12 intracerebral hemorrhage patients diagnosed by CT were operated with improved vessel sheath, and the effects as well as complications of the operation were evaluated. Results Operations in all cases were successful. Hematoma of 6 cases was eliminated incompletely and therefore the direction of vessel sheath was adjusted. The average drainage volume of hematoma was 85%, and the vessel sheath was pulled out at about 7.5 days in average. 2 cases were dead, one died due to choke and the other clue to kidney function failure, and all complications had nothing to do with operations. Conclusion Application of improved vessel sheath on the operation of intracerebral hemorrhage patients is feasible, safe and effective. Its strongpoint is its plasticity and noncounterfeit image.
