摘    要:目的 探讨电针足三里对牛黄胆酸钠诱导的重症急性胰腺炎(SAP)大鼠炎症反应及肠道免疫功能的影响。方法 将72只SD雄性大鼠随机分为电针组、模型组、假手术组,每组24只。通过逆行胰胆管注射3.5%牛磺胆酸钠制作SAP模型。电针组于模型制作成功后和处死前,固定在操作台上给予双侧“足三里”穴电针治疗各30 min。模型组及假手术组仅固定30 min不针刺。各组大鼠分别于造模成功后3、6、12 h分批处死,用酶联免疫吸附法测定大鼠血清白细胞介素-4(IL-4)、白细胞介素-10(IL-10)、转化生长因子-β1(TGF-β1)和干扰素-γ(IFN-γ)的水平,苏木精和伊红染色观察各组胰腺组织、小肠组织病理变化并进行评分,并用免疫组化法检测小肠分泌型免疫球蛋白A(SIgA)的表达水平。结果 电针组术后3、6、12 h血清IL-4、IL-10、IFN-γ含量均低于模型组(P <0.05),血清TGF-β1含量均高于模型组(P <0.05);假手术组及电针组各时间点胰腺病理评分均低于模型组(P <0.05);假手术组各时点小肠病理评分明显低于模型组(P <0.05),电针组小...

Abstract:Objective:To investigate the effect of electroacupuncture at Zusanli(ST36)on inflammatory response and intestinal immune function in rats with severe acute pancreatitis(SAP)induced by sodium taurocholate.Methods:A total of 72 male SD rats were randomly divided into the electroacupuncture group(EA group),model group(SAP group)and sham operation group,with 24 rats in each group.SAP model was established by retrograde injection of 3.5%sodium taurocholate into the pancreaticobiliary duct.The EA group was fixed on the operating table and given bilateral Zusanli(ST36)after successful model establishment and before execution.Each point was treated with electroacupuncture for 30 minutes.The SAP and sham operation groups were only fixed for30 minutes without electroacupuncture.Rats in each group were sacrificed in batches of 3,6,and 12 h after successful modeling;serum levels of interleukin-4(IL-4),interleukin-10(IL-10),transforming growth factor-β1(TGF-β1),and interferon-γ(INF-γ)were measured by enzyme-linked immunosorbent assay.Pancreatic and small intestinal histopathological changes in each group were observed and scored by hematoxylin and eosin staining,and small intestinal secretory immunoglobulin A(SIg A)expression was detected by immunohistochemistry.Results:The serum levels of IL-4,IL-10,and IFN-γin the EA group at 3,6,and 12 h after operation were lower than those in the SAP group(P<0.05);the TGF-β1 level in the EA group at each time point was higher than that in the SAP group(P<0.05);the pancreatic pathological scores of the sham and EA groups at each time point were lower than those of the SAP group(P<0.05);the pathological score of the small intestine in the sham operation group was significantly lower than that in the SAP group at each time point(P<0.05).There was no significant difference in the pathological score of the small intestine between the EA group and the SAP group at 3 hours,but it was lower than that in the SAP group at other time points,and the difference was statistically significant(P<0.05);SIg A expression in the small intestine in the sham operation group and EA group was higher than that in the SAP group(P<0.05).Conclusion:Electroacupuncture at Zusanli can regulate TGF-β1 and IFN-γ.The expression level of SIg A protein in the small intestine is up regulated to protect the intestinal mucosal barrier,prevent bacteria and endotoxin from entering the blood,reduce inflammatory reaction and improve the prognosis of SAP.
