通腑泄肺汤对急性呼吸窘迫综合征肠黏膜屏障功能、炎症因子及肺动态顺应性的影响

目的观察通腑泄肺汤对急性呼吸窘迫综合征(ARDS)肠屏障功能、炎症因子及肺动态顺应性的影响。方法ARDS患者62例采用随机数字表法分为治疗组与对照组各31例。对照组采用内科常规治疗,治疗组在内科常规治疗的基础上采用通腑泄肺汤治疗,两组均治疗7 d。比较两组治疗后临床症状评分、炎症因子水平、氧化应激、肠屏障功能、呼吸指数、肺动态顺应性、呼吸指数及不良反应情况。结果治疗后两组临床症状评分、炎症因子水平、氧化应激、肠屏障功能指标及呼吸指数均低于治疗前(P<0.05),且治疗组上述指标低于对照组(P<0.05);治疗后两组肺动态顺应性高于治疗前(P<0.05),且治疗组肺动态顺应性高于对照组(P<0.05);治疗期间两组患者均未出现不良反应。结论通腑泄肺汤治疗ARDS能够减轻临床症状及氧化应激损伤,改善肠黏膜屏障及肺通气功能,抑制炎症因子表达,提高肺动态顺应性且具有较好的安全性。

Effect of Tongfu Xiefei Decoction on Intestinal Mucosal Barrier Function,Inflammatory Factors and Lung Dynamic Compliance in Acute Respiratory Distress Syndrome

Objective:To observe the effects of Tongfu Xiefei Decoction on intestinal barrier function,inflamma?tory factors and lung dynamic compliance in acute respiratory distress syndrome(ARDS).Methods:A total of 62 patients with ARDS were divided into 2 groups according to random number table method,31 cases in each group.The control group received routine medical treatment,and the treatment group was treated with Tongfu Xiefei De?coctionon the basis of routine medical treatment.Both groups were treated for 7 days.The clinical symptom scores,inflammatory factor levels,oxidative stress,intestinal barrier function,respiratory index,lung dynamic compliance,respiratory index and adverse reactions were compared between the two groups after treatment.Results:After treatment,the clinical symptom scores,inflammatory factor levels,oxidative stress,intestinal barrier function index?es and respiratory index of the two groups were lower than before treatment(P<0.05),and the above indexes of the treatment group were lower than those of the control group(P<0.05).The lung dynamic compliance of the two groups after treatment was higher than that before treatment(P<0.05),and the lung dynamic compliance of the treatment group was higher than that of the control group(P<0.05).During the treatment,there were no adverse reactions in the two groups.Conclusion:Tongfu Xiefei Decoction for ARDS can reduce clinical symptoms and oxi?dative stress damage,improve the intestinal mucosal barrier and lung ventilation function,inhibit the expression of inflammatory factors,improve lung dynamic compliance and has good safety.
