缪刺法联合中药熏洗治疗脑卒中后肩手综合征的疗效观察

目的:观察缪刺法联合中药熏洗治疗脑卒中后肩手综合征（SHS）的疗效。方法:选取2021年1月至2021年8月北京中医药大学房山医院月华分院康复科收治的脑卒中后SHS患者100例作为研究对象，按信封简单随机法1:1分为观察组和对照组，每组50例。观察组采用缪刺法联合中药熏洗治疗，对照组采用常规针刺法联合中药熏洗治疗。评价2组患者患侧上肢疼痛情况、运动功能及肿胀程度。结果:2组患者视觉模拟评分法（VAS）评分、患手肿胀评分、Fugl-Meyer运动功能评分量表评分、巴塞尔指数评分均优于治疗前（P<0.05），治疗2周和4周观察组患者VAS评分、Fugl-Meyer运动功能评分量表评分、巴塞尔指数评分均优于对照组，观察组治疗4周后在改善疼痛、患手肿胀程度、运动功能及提高日常生活能力效果最明显（P<0.05）。观察组患者治疗总有效率为94.0%，对照组患者治疗总有效率为76.0%，差异有统计学意义（P<0.05）。结论:缪刺法联合中药熏洗可减轻卒中后SHS患者患肢疼痛，提高运动功能并缓解肿胀程度。

Effect of Contralateral Needling Therapy Combined with Herbal Fumigation on Shoulder-hand Syndrome after Stroke

To observe the effect of contralateral needling combined with herbal fumigation on shoulder-hand syndrome(SHS) after stroke.Methods:A total of 100 patients with SHS after stroke admitted to the Rehabilitation Department of Yuehua Branch of Fangshan Hospital Beijing University of Chinese Medicine from January 2021 to August 2021 were selected as the research objectives.The patients were divided into an experimental group and a control group by envelope simple random method,with 50 patients in each group.The experimental group was treated with contralateral needling therapy combined with herbal fumigation,and the control group was treated with conventional acupuncture combined with herbal fumigation.The pain degree,motor function,and edema degree of the affected upper limbs of patients in the two groups were observed.Results:After treatment,the Visual Analogue Scale(VAS) score,hand edema degree score,Fugl-Meyer Assessment(FMA) score,and Barthel Index(BI) score of the two groups were improved(P<0.05).After treatment for 2 and 4 weeks,the VAS score,FMA score,and BI score in the experimental group were better than those in the control group.Compared with the control group,the experimental group obviously improved the pain and edema degree,motor function,and ability of daily living(ADL)(P<0.05) after treatment for 4 weeks.The total effective rates in the experimental group and the control group were 94.0% and 76.0%,respectively(P<0.05).Conclusion:The contralateral needling therapy combined with herbal fumigation relieves the pain and edema degree,and improves motor function and ADL of patients with SHS after stroke.
