上海市癌症长期生存者牙齿缺失及口腔卫生状况对生命质量的影响

摘    要:目的 探讨癌症长期生存者生命质量与牙齿缺失及口腔卫生状况之间的关系。方法 2013年4-7月在上海市采用整群抽样法对5 623名癌症长期生存者进行问卷调查,调查内容包括社会人口学情况、肿瘤的诊断和治疗情况、口腔健康状况(牙齿缺失数、刷牙频次、牙齿检查)等,采用癌症治疗功能评价系统量表体系中的共性模块(Functional Assessment of Cancer Therapy-General,FACT-G)评估癌症生存者的生命质量。采用多重线性回归模型评估FACT-G各维度生命质量得分在不同牙齿缺失数、刷牙频次、牙齿检查上的均数差(mean difference,MD)和95%可信区间(95% CI)。结果 17.5%的癌症长期生存者牙齿缺失超过10颗,57.05%的癌症长期生存者能坚持每天至少刷牙2次,仅7.7%的癌症长期生存者坚持至少每年进行1次牙齿检查。FACT-G各维度得分和量表总分随着牙齿缺失数量的增加而减少(Ptrend均<0.05)。刷牙≥ 2次/天的癌症长期生存者FACT-G量表总分均高于刷牙≤ 1次/天的癌症长期生存者。每年至少进行1次牙齿检查的癌症长期生存者的社会状况、功能状况维度得分和FACT-G量表总分均高于不能每年进行牙齿检查的癌症长期生存者。结论 牙齿缺失数、刷牙频次和牙齿检查与上海市癌症长期生存者的整体生命质量存在关联,良好的口腔健康状况和科学的口腔健康行为可能有助于改善癌症生存者的生命质量。

Abstract:Objective To investigate the effect of tooth loss and oral hygiene status on quality of life among long-term cancer survivors in Shanghai. Methods A total of 5 623 long-term cancer survivors were selected by cluster sampling from April to July 2013 in Shanghai,China.Data were collected using a self-reported questionnaire,which included questions about basic socio-demographic characteristics,cancer conditions and treatments,and oral health status (number of tooth loss,frequency of tooth brushing and dental visit).Quality of life was measured using the Functional Assessment of Cancer Therapy-General (FACT-G).Multiple linear regression models were performed to estimate the mean differences and 95% CI of quality of life scores. Results Of the long-term cancer survivors,17.5% reported more than 10 missing teeth,57.05% brushed tooth at least twice a day,only 7.7% lreported dental visit at least once a year.With the increase of the number of missing tooth,participants reported lower FACT-G dimension scores and total score (Ptrend<0.05).Participants with at least twice a day toothbrushing reported higher FACT-G total score than those with at most once a day toothbrushing.Participants with dental visit at least once a year reported higher social/family well-being, functional well-being and FACT-G total score than those with zero dental visit. Conclusion Number of tooth loss, frequency of toothbrushing and dental visit were associated with the overall quality of life among long-term cancer survivors in Shanghai. Oral health and scientific oral health behaviors may be helpful to improve the quality of life of cancer survivors.
