70例脑梗患者CYP2C19基因多态性与中医证型分布的相关性

摘    要:目的: 探究脑梗患者CYP2C19*2、CYP2C19*3和CYP2C19*17基因多态性与中医证型分布的相关性。方法: 将我院2017年4月-2019年7月中医科的脑梗患者根据中医证型进行分型,并且采集其静脉血,提取基因组DNA,然后进行扩增及结果判定,观察CYP2C19*2CYP2C19*3CYP2C19*17基因位点多态性与中医证型分布的相关性。结果: 某院中医科的脑梗患者根据中医分型,分为阴虚风动型、气虚血瘀型、痰热腑实型和风痰瘀阻型,其中,气虚血瘀型占比50%,占比明显高于其他证型(P<0.05)。基因检测分析发现,脑梗患者CYP2C19基因的突变易发于2、3位点,CYP2C19*2突变等位基因频率为0.307 2;CYP2C19*3突变等位基因频率为0.085 7;CYP2C19*17突变等位基因频率为0.021 5。其中,气虚血瘀型患者中CYP2C19*2基因突变率最高,并且CYP2C19*2突变型比率高于野生型(P<0.05)。结论: 脑梗患者的中医证型分布与CYP2C19*2基因多态性具有显著的相关性;气虚血瘀证型为脑梗患者的主要分型,并且气虚血瘀证型脑梗患者大多数存在CYP2C19*2基因突变,并且呈正相关。

Abstract:OBJECTIVE To explore the correlation between polymorphisms of CYP2C19*2, CYP2C19*3 and CYP2C19*17 genes and distribution of TCM syndromes in patients with cerebral infarction.METHODS The patients with cerebral infarction in our hospital from April 2017 to July 2019 were classified according to TCM syndromes, and their venous blood was collected to extract genomic DNA. Then the genomic DNA was amplified and the results were determined. The correlation between CYP2C19*2, CYP2C19*3 and CYP2C19*17 polymorphisms and the distribution of TCM syndromes was observed.RESULTS According to the classification of TCM, the patients with cerebral infarction in our hospital were divided into Yin deficiency wind-driven type, Qi deficiency and blood stasis type, phlegm-heat-viscera type and wind-phlegm-blood stasis type. Among them, the proportion of Qi deficiency and blood stasis type was 50%, which was significantly higher than other types (P<0.05). Gene analysis showed that CYP2C19 gene mutation was easy to occur at loci 2 and 3 in patients with cerebral infarction. The allele frequency of CYP2C19*2 mutation was 0.307 2; the allele frequency of CYP2C19*3 mutation was 0.085 7; and the allele frequency of CYP2C19*17 mutation was 0.021 5. Among them, the mutation rate of CYP2C19*2 gene was the highest in patients with Qi deficiency and blood stasis, and the mutation rate of CYP2C19*2 was higher than that of wild type (P<0.05).CONCLUSION The distribution of TCM syndromes in patients with cerebral infarction is significantly correlated with CYP2C19*2 gene polymorphism. Qi deficiency and blood stasis are the main types of cerebral infarction, and most of the patients with Qi deficiency and blood stasis syndrome have CYP2C19*2 gene mutation, which is positively correlated.
