胃肠道间质瘤的超声诊断

目的通过总结胃肠道间质瘤(GIST)的超声表现,评价超声在其诊断中的价值。方法对58例GIST的超声表现和超声诊断与病理诊断结果作对照分析。结果病理诊断良性和交界性间质瘤19例,恶性间质瘤39例。超声检查漏诊8例,检出50例,其中27例诊断为GIST。在GIST的超声表现中,恶性病变的大小、内部回声、轮廓有无分叶与良性病变有显著差别(P<0.01),而肿瘤边缘与边界、彩色血流丰富程度在良、恶性病变间无明显差别(P>0.01)。结论经腹超声有助于GIST的检出。肿瘤较大、回声不均、分叶生长是提示恶性间质瘤的重要超声征象。

The Value of Ultrasound in Diagnosing of Gastrointestinal Stromal Tumors

Objective The aim of this study was to define the imaging characteristics of primary gastrointestinal stromal tumors (GIST) in ultrasound and evaluate the value of ultrasound in diagnosis of GIST.Methods The medical records of 58 patients with a histologic diagnosis of GIST were reviewed.The sonographic manifestations were analyzed retrospectively and diagnosis of ultrasound were correlated with those of pathology.Results Of 58 neoplasms including the malignant (n=30),borderline (n=3) and benign GISTs (n=16),Transabdominal ultrasound detected 50 lesions and diagnosed 27 as GIST and missed 8 lesions.The size,internal echo pattern and the contour of malignant GISTs were statistical significant different from those of the others (P<0.01),the margin and border and the color Doppler flow signal were not significant different between malignant GISTs and the others (P>0.01).Conclusions Transabdominal ultrasound is a useful method to detect GIST.The ultrasonographic features that suggest malignant GIST include large size,heterogenous echo pattern and lobulated contour.
