早产低体质量及极低体质量儿的麻醉管理

目的对临床早产低体质量及极低体质量新生儿进行麻醉管理的方法进行总结性研究。方法抽取在本院出生的46例接受手术治疗的早产低体质量和极低体质量新生儿病例，根据患儿具体病情与具体状况确定麻醉方法，分为静脉麻醉21例，复合全麻气管插管麻醉25例。比较两组患儿的手术成功率及并发症情况。结果复合全麻组患儿手术成功24例，手术成功率为96．0％；静脉麻醉组患儿手术成功19例，手术成功率为90．5％。两组患儿手术成功率、麻醉并发症比较差异有统计学意义（P〈0．05）。结论采用以吸入麻醉剂为主的复合式麻醉方法对早产低体质量及极低体质量新生儿进行麻醉可起到良好的麻醉效果，且并发症较少，值得推广应用。

Anesthesia management for premature low-weight and premature extreme low-weight newborns

Objective To do the concluding study for the anesthesia management ways of clinical premature low-weight newborns and premature extreme low-weight newborns.Methods The 46 cases were taken of which were the premature low-weight newborns and premature extreme low-weight newborns who were born in our hospital and had taken surgery.The anesthesia ways were defined according to the newborns’ specific states of illness and the specific conditions,which were 21 cases with bier’s anesthesia and 25 cases compound general tracheal intubation anesthesia.The success rate and the complications of the newborns from 2 groups were compared.Results There were 24 cases successful surgery of the newborns from the compound general anesthesia group,and the surgery success rate was 96.0%;There were 19 cases successful surgery of the newborns from the bier’s anesthesia group,and the success rate was 90.5%.Two groups of children the success rate,anesthesia complications comparing statistical significance (P< 0.05).Conclusion The compound anesthesia method mainly adopting inhalation anesthetics has a good anesthesia effect on the premature low-weight and premature extreme low-weight newborns with less complications.It is worth being widely applied.
