高血压患者社区防治中健康管理的运用探析

目的评价健康管理在高血压社区防治中的效果。方法选取2011年1～7月间200例高血压患者,将其分为治疗组和对照组,治疗组主要通过一般的护理干预,而对照组则进行相关健康知识和行为指导,发放相关宣传资料,并派护士对患者进行为期半年的健康管理跟踪等。结果经过SPSS13.0分析可知,健康管理前后两组血压值和血压控制率比较差异无统计学意义。但健康管理后与健康管理前相比较,研究对象血压均值水平整体上有所下降。结论在社区高血压管理中健康管理模式已有初步效果,同时对高血压患者相关健康知识的学习和行为的改善具有重要作用,有利于更好地降低高血压危险因素的流行率。

Discussion on the applying of health management in the prevention and control of hypertension in community

Objective To evaluate the health effects of health management in the prevention and control of hypertension in community.Methods Two hundred patients with hypertension selected from January to July 2011 were divided into treatment group and control group,the patients in treatment group were mainly given general nursing interventions,while the patients in control group were given health knowledge and behaviors guidance who were given the related publicity materials,the health management of control group were followed up by their nurses in a period of six months and so on.Results After SPSS 13.0 analyzed,the blood pressure and blood pressure control rates of two groups showed no significant difference before and after health management,there was no statistical significance.Before health management compared with after health management,the blood pressure of patients generally declined.Conclusion The model of health management has had primary effects in the management of hypertension in community,at the same time,it has an important effect to make patients with hypertension study the relevant health knowledge and improve their behavior,in order to reduce the rate of prevalence of risk factors of hypertension.
