摘    要:目的:探讨系统的排痰护理,减少有创机械通气患者痰阻发生,改善通气的疗效。方法:选择本院2007年10月~2010年12月收治的行有创机械通气的患者108例,随机分为研究组和对照组组,各54例,对照组给予常规的机械通气排痰护理,研究组给予系统的排痰护理包括增加室内湿度、湿化气道、掌握吸痰时机和吸痰技巧、保证机体液体入量等,观察两组护理效果及通气改善情况。结果:对照组痰阻发生率明显高于研究组,差异有统计学意义(P〈0.05)。两组死亡例数比较差异无统计学意义(P〉0.05),两组机械通气后血氧分压比较,差异无统计学意义,但是对照组血二氧化碳分压明显高于研究组,差异有统计学意义。结论:科学系统的的排痰护理措施能有效减少有创机械通气患者痰阻发生,改善通气效果。

Abstract:Objective: To investigate the effect of expectoration care in reducing phlegm and improving the ventilation of patients with invasive mechanical ventilation. Methods: One hundred and eight patients with invasive mechanical ventilation in our hospital from October 2007 to December 2010 were randomly divided into study group and control group, 54 cases in each group, patients in control group were given conventional expectoration care, patients in study group were given systematic expectoration care including increase indoor humidity, airway humidification, control the suction timing and suction techniques, ensure fluid intake, etc.. Care effect and ventilation improvement of two groups were observed. Results: Phlegm of control group were significantly more than study group, the difference was statistically significant (P<0.05). Death cases of two groups had no statistically significant difference (P>0.05), PaO2 of two groups had no significant difference, but PaCO2 of the control group was significantly higher than the study group, the difference was statistically significance. Conclusion: The scientific systemanic expectoration care can effectively reduce phlegm in patients with invasive mechanical ventilation, and it can improve the ventilation effect.
