嵌顿痔的临床病理研究

[目的]探讨嵌顿痔的病因病理及痔的本质。[方法]收集2000年10月至2002年1月痔手术切除标本中嵌顿痔30例，混合痔42例，在光镜观察其微结构变化，两组各取10例在电镜下观察超微结构变化。[结果]嵌顿痔的血管、结缔组织、肌组织和问质等都发生变化，其中以血管及纤维的变化尤为明显，并间质高度水肿，有急慢性炎症细胞浸润；嵌顿痔组血管扩张程度明显大于混合痔组(P＜0．01)，而炎症细胞浸润的数量、种类和深浅度与混合痔组无差异(P＞0．05)。[结论]嵌顿痔是痔病理发展的严重阶段，嵌顿痔的病理变化与混合痔大致相仿，其并非由细菌感染引起。

Clinical Study on Aetiology and Pathology of incarceration of Internal Hemorrhoids IIH

To investigate the pathogenesis of IIH.The specimens of 30 case of IIH(IIH group) and 42 case of mixed hemorrhoids after hemorrhoidectomy from October 2000 to January 2002 were collected;to anlayze the histopathology under the optical microscope,Therein 10 cases of each group wer taken to observe the change in ultrastructure under the electron microscope.It was fond that there were changes on blood vessels and the connective tissue and the muscular tissue and the interstitial of IIH,especially in the blood vessels and fibers,and serious edema in the interstitial,as well as active chronic inflammatory cell infiltration.The degree of vascular dilatation of IIH group was greater than that of MH group(P<0.05),but there were no difference in the quantity,the variety and the depth of inflammatory cell infiltration in two groups(P>0.05).[Conclusion]IIH is the serious pathological stage of hemorrhoids,the pathological changes of IIH group is similar to that of MH group,IIH isn't caused by bacterial infection.
