单枚cage置入结合单侧侧后方植骨融合加椎弓根螺钉固定治疗腰椎滑脱症

目的探讨单枚cage斜向置入结合对侧侧后方植骨融合加椎弓根螺钉固定治疗腰椎滑脱症的临床疗效。方法对42例腰椎滑脱患者行后路减压和椎弓根螺钉固定,单侧斜向放置单枚cage,对侧侧后方植骨融合治疗,随访观察患者的症状、体征改善程度、融合率和手术节段椎间隙高度的变化。结果所有患者均获得随访,时间2年～4年5个月,临床疗效综合评价：优32例,良5例,可3例,差2例。症状改善率92.9%。患者满意程度为88%,融合率为90.5%,最终随访时椎间隙高度平均减少1.5mm。未出现螺钉折断和明显的复位丢失现象。结论单枚cage置入结合对侧侧后方植骨融合加椎弓根螺钉固定治疗腰椎滑脱症对后柱破坏少,具有较高稳定性。

Treatment of lumbar spondylolisthesis with transpedicle instrument fixation system, mono-cage and semi-intertransverse process arthrodesis with bone grafting

Objective To observe the outcomes of lumbar spondylolisthesis treated with transpedicle instrument fixation system, mono-cage and semi-intertransverse process arthrodesis with bone grafting. Methods 42 cases of lumbar spondylolisthesis were treated with transpedicle instrument fixation system, mono-cage and semi-intertransverse process arthrodesis with bone grafting. The follow-up study included clinical outcomes, heights of intervertebral space and foramen, fusion rate and screw breakage rate. Results All patients were followed up for 2 - 3 years and 5 months with an average of 34 months. The results showed that the pre-operative clinical symptoms disappeared completely in 39 of 42 patients. Pain relief was seen in 92. 9% ; 37 of 42 patients were satisfied; Solid fusion was obtained in 90. 5% ; At the last follow-up, the average loss of intervertebral height was 1.5 mm; No screw breakage or displacement was found. Conclusions Treatment of lumbar spondylolisthesis with transpedicle instrument fixation system, mono-cage and semi - intertransverse process arthrodesis with bone graft can provide good stability with minimal damage.
