产科联合介入麻醉救治前置胎盘合并胎盘植入25例临床分析

目的探讨多学科联合处置前置胎盘合并胎盘植入的方法。方法回顾性分析25例前置胎盘合并胎盘植入,其中膀胱植入5例。介入科负责经股动脉放置导管至子宫动脉,麻醉科除负责麻醉外,开放中心静脉,建立有创动脉检测。统计生命体征变化、住院天数、产后出血和子宫切除数。结果产科联合介入麻醉科处置前置胎盘合并胎盘植入,较以前传统方法生命体征平稳,减少产后出血、子宫切除数及住院天数。结论多学科联合救治前置胎盘合并胎盘植入,减少产后出血,减少子宫切除及住院时间,维持围术期生命体征平稳,提高安全系数,值得推广应用。

Clinical Analysis of Obstetrics Combined with Intervention and Anesthesiology in Treatment of 25 Cases with Placenta Previa and Placenta Accreta

Objective To discuss the multidisciplinary combined treatment of placenta previa and placenta increta. Methods Retrospective analysis was taken for 25 patients of placenta previa and placenta increta, including 5 patients of bladder implanted. Intervention department was responsible for placing catheter to uterine artery through femoral artery, anesthesiology department was responsible for not only anesthesia, but also opening central vein and setting up artery pressure. The changes of vital signs, days in hospital, postpartum hemorrhage and hysterectomy numbers were recorded. Results The combined intervention with anesthesiology and obstetrics departments in the treatment of placenta previa and placenta increta had more stable vital signs than the traditional method, reduced days in hospital, postpartum hemorrhage and hysterectomy numbers. Conclusion Multidisciplinary combined treatment of placenta previa and placenta increta can reduce postpartum hemorrhage, hysterectomy numbers, and days in hospital, maintain perioperative vital signs stable and improve safety coefficient, which is worthy of application.
