保胆取石在基层医院应用的报告(附32例报告)

目的探讨保胆取石在基层医院推广应用的前景。方法回顾性分析自2010年6月至2011年8月采用保胆取石方法治疗的32例胆囊结石患者的病例资料,与同期因胆囊结石行腹腔镜胆囊切除术的78例的患者进行手术时间、手术出血量、肠功能恢复时间、手术并发症、手术费用及住院时间方面进行比较。结果两组在手术时间、手术并发症及手术费用方面比较有显著性差异;保胆取石组随访无复发,腹腔镜胆囊切除术组出现继发性胆管结石2例,EST处理后取尽结石。结论保胆取石治疗胆囊结石患者在选取合适的适应症下是一种安全可靠的方法,在基层医院有良好的推广应用价值。

A Practical Report on Cholecystolithotomy with Preservation of Gallbladder in Local Hospitals: Report of 32 Cases

Objective To explore the application prospect of cholecystolithotomy with preservation of gallbladder in local hospitals. Methods The clinical data of 32 cholecyslithiasis patients treated with cholecystolithotomy with preservation of gallbladder and 78 patients with laparoscopic cholecystectomy (LC) from June 2010 to August 2011 were analyzed retrospectively. The operation time, bleeding volume during operation, recovery time of enterokinesia, post-operative complications, surgery cost and hospital stay were compared. Results There were significant differences in the operation time, post-operative complication and surgery cost between the two groups. There was no recurrence of stones in patients with cholecystolithotomy with preservation of gallbladder, 2 cases of secondary choledocholithiasis in LC group and healed after EST. Conclusions Cholecystolithotomy with preservation of gallbladder is a safe and reliable method in the treatment of cholecyslithiasis based on choosing correct surgical indications, which has good application value in local hospitals.
