高血压脑出血三种手术治疗的临床体会

目的探讨高血压脑出血的三种手术治疗方法。方法根据出血量大小以及GCS评分,对大骨瓣开颅清除血肿、小骨窗开颅血肿清除术及微创血肿穿刺碎吸术治疗高血压脑出血进行临床应用的比较。结果本组96例患者出院时GOS分级:I级12例,Ⅱ级14例,Ⅲ级32例,Ⅳ级13例,V级9例,死亡16例。结论三种手术各有其相对应的适应证,根据患者病情选择不同手术方式治疗高血压脑出血对于术后患者病情恢复、减少并发症有重要意义。

Clinical Experience on Three Kinds of Surgical Treatment for Hypertensive Intracerebral Hemorrhage

Objective To explore three kinds of surgical treatment for hypertensive intracerebral hemorrhage. Methods The clinical application of three surgical treatments for hypertensive intracerebral hemorrhage including clearance of hematoma by craniotomy with great bone flap, small bone window and minimally invasive puncture were compared according to amount of bleeding and GCS score. Results The GOS classifications at discharge in all 96 patients were as follows: 12 cases of level I, 14 cases of level Ⅱ, 13 cases of level Ⅳ, 9 cases in level V and 16 cases of death. Conclusions Three kinds of surgical treatment have its corresponding indication; choosing appropriate surgical modality for hypertensive intracerebral hemorrhage according to physical condition is important to patients’ recovery and reducing the complications.
